Portage North 2009 Reschedule Game :: Class F (13-14 Boys)
Instructions
Please use this form to reschedule a game. Enter the original Schedule Date.
The Home Team select box will be populated with the teams that have a home game
scheduled on the selected date.
Enter the Resched Date on which the game will be played.
Select the Home Team to display the Visiting Team and populate the
Field selection box with valid field names.
Select the Field and the Time.
Enter the Name and Phone of the team manager.
Required fields are marked with an *. The form will
not be submitted until all required fields are entered. When Submit is selected,
captions of required fields not entered are changed to a red bold font to identify the
missing information.
The information entered on this form is emailed to the Portage North. If you have any
questions or problems, contact the Scheduling Coordinator.
NOTE your IP address is captured as part of the email sent to Portage North.
